Lp(a): pelacarsen lowers the marker, not the heart risk

Pelacarsen was meant to prove that lowering Lp(a), an inherited cardiac risk factor carried by one person in five, prevents heart attacks and strokes. The global Lp(a)HORIZON trial of 8,323 patients answers no: the marker falls, the events remain. That gap now questions a whole generation of treatments.

The fact

One adult in five worldwide inherits a high level of Lp(a), a blood particle that raises heart-attack risk and that diet and exercise barely shift. On September 4, Novartis announced that pelacarsen, the first dedicated therapy to complete a large outcomes trial, did lower Lp(a) in 8,323 cardiac patients, without reducing their cardiovascular events: deaths from cardiovascular causes, non-fatal heart attacks and strokes, urgent coronary revascularizations.

Why it deserves attention

Lp(a) levels are about 90% set by genes. Genetic studies point to that level as a cause of the risk, not merely a marker, which is what convinced drugmakers to design molecules that slash the number. Lp(a)HORIZON tested the missing link: is lowering the level enough to prevent the events? The first answer is no, at least in patients who are already ill and already on statins and blood-pressure drugs.

Two explanations dominate. Lp(a)-related damage builds up over decades, so a late correction may not undo what is already installed. And in well-treated patients, the remaining margin of benefit becomes thin. The stakes reach beyond Novartis: Amgen and Eli Lilly are testing their own Lp(a)-lowering drugs, and their shared assumption, that a marker which predicts risk is a lever you can pull, is now shaking. For a fifth of humanity, the first targeted treatment stays out of reach, and the proof now rests on those rival trials.
